  • Once the conditions of a permit are agreed to, the permit holder can sign the permit and shall become the responsible person for supervising the task.

Working Under a Permit to Work

  • The permit holder must monitor the health and safety works in accordance with the scope and agreed on control measures with the understanding that should any conditions change or any unexpected hazards or risks be identified that work must stop and be re-assessed.
  • Any changes that affect the validity of a permit will result in the immediate cancelation of the permit. A new permit to work must then be issued for the works in this circumstance.
  • If a permit to work requires transfer from one permit holder to another, all required signatures on the transferring the permit section must be completed.
  • Permit authorisers must ensure by inspection that all works associated with the permit to work have been completed correctly and that the work area has been re-instated to safe operations once the work outlined in the permit has been completed.

Note: All permits should be invalidated in the event of an emergency and all works should stop immediately. All permits should be re-checked and revalidated once an emergency is over and before work recommences.
