How to Complete a Permit to Work
A hazard identification assessment must be undertaken by an authorised person on whether a work permit is required for the work activities. All relevant information must be made available to the permit authoriser by the person completing the permit.
A permit to work must comply with the following before permitted work can commence:
- All details on the permit to work system that are relevant to the task are completed.
- The associated documentation (drawings, checklists, etc) are attached for review by the permit authoriser.
- The contents of a permit, specifically the permit conditions and potential hazards, must be communicated by the permit holder to the workgroup to enable consultation and feedback.
- The permit holder must ensure that the workgroup fully understands the scope of work and conditions and sign onto the permit to confirm this understanding.
- No person should simultaneously hold the positions of permit holder and permit authoriser.
- Once the conditions of a permit are agreed to, the permit holder can sign the permit and shall become the responsible person for supervising the task.
Working Under a Permit to Work
- The permit holder must monitor the health and safety works in accordance with the scope and agreed on control measures with the understanding that should any conditions change or any unexpected hazards or risks be identified that work must stop and be re-assessed.
- Any changes that affect the validity of a permit will result in the immediate cancelation of the permit. A new permit to work must then be issued for the works in this circumstance.
- If a permit to work requires transfer from one permit holder to another, all required signatures on the transferring the permit section must be completed.
- Permit authorisers must ensure by inspection that all works associated with the permit to work have been completed correctly and that the work area has been re-instated to safe operations once the work outlined in the permit has been completed.
Note: All permits should be invalidated in the event of an emergency and all works should stop immediately. All permits should be re-checked and revalidated once an emergency is over and before work recommences.
